We are honored that you have trusted Pediatric Partners in Winter Haven, Florida to care for your child. As with any new patient registration, there is plenty of patient info and paperwork to fill out at the time of the initial visit. However, to cut down on the time spent at our office with your little one, you may view, print and fill out necessary new patient forms beforehand and bring them with you when you come to our Pediatric Clinic in Winter Haven. Please be sure to arrive at least 20 minutes early for your child’s first visit and have the following items with you:
- Photo ID of Parent of Guardian
- Insurance Card or Printed Insurance Information for New Patient
- New Patient Forms (Optional: You may fill out beforehand for ease or at the time of visit)
- Applicable Shot Record for Patient (Can be obtained from existing physician and you may bring it with you to your visit or you may fax it to us directly at 863-293-3732)
It is helpful if you arrive 20 minutes prior to your scheduled appointment time to allow our staff to get your child ready for his/her visit. Children can be very unpredictable which complicates our schedule on a daily basis so we ask for your patience in advance. When it is your turn for your child to be seen by our caring pediatrician, we will provide your child the same undivided attention as we provide to previously scheduled patients.
