Pediatric Partners Makes Required Examinations Easy
Team sports, various organizational camps and schools may require a physical screening from each participant or student previous to attendance. It’s important to understand that sports and camp physicals are different from annual checkups, but are similarly designed to determine any physical limitations and, if necessary, treatment to ensure safety.
Typically, you should first obtain a form from the organization requiring the physical. Initially, there are some items on the form that must be filled out by the parent or guardian of the participant. This form is necessary for the examination and must be presented at the time of your scheduled appointment.
The health information that must be documented is noted on the form and guides our Providers on the medical information they must obtain during the examination.
In addition, if you are not a current patient of Pediatric Partners, we suggest that you bring official immunization records with you to the exam.
